Delores admitted to The Woodlands after being hospitalized for a fall and a urinary tract infection.
At the time of her admission, she required maximum assistance with bed mobility and transfers, and she was unable to walk or dress without support.
Over the course of just six weeks, Delores underwent physical and occupational therapies five times a week. She had three major therapy goals to achieve: strengthening both her upper and lower extremities, improving her transferring ability, and enhancing endurance for walking.
Sure enough, Delores reached all her recovery goals and is now discharging home to live with her daughter. She has made significant progress and is now able to transfer with minimal assistance, dress with supervision, and walk up to 125 feet using a walker, with some standby help.

Mr. Godfrey, affectionately known as "Coach Buck," is a highly respected retired coach in the Atlanta area. He is best known for guiding one of the longest winning streaks in football at a well-known high school.
Upon admission to Life Care Center of Gwinnett, he required max assistance with all mobility, self-care tasks, and transfers. He could walk only 15 feet with a walker and moderate assistance. Use of both his arms was very limited.
To meet his goals in physical therapy and occupational therapy, Coach Buck had to rely on the same virtues he instilled in his players –– initiative, hard work, commitment, and perseverance. He worked in therapy five days a week and focused on training to improve his mobility, gait, and ability to complete self-care tasks.
Coach Buck is now at supervision and setup assist level with mobility, self-care tasks, and walking. He proudly walked out of this facility with a cane, escorted by his son, to a cheering staff. He returned home to his wife.

After a hip replacement, Angeles was not ready to return home, so she agreed to go to Life Care Center of Yuma for short-term rehabilitation services.
When Angeles first arrived, she was totally dependent and needed maximum assistance with all tasks, including bathing, dressing, toileting, transferring, and walking.
After the therapists assessed her, Angeles received physical, occupational, and speech therapies five days a week for about 35 minutes a day. Her goals were to improve her upper and lower body strength, walking ability, self-care tasks, and safety techniques. She also practiced swallowing techniques in speech therapy.
Angeles has met all her goals and can complete tasks independently or with some setup help. She is now walking with an adaptive device, dressing in her own, speaking more clearly, swallowing better, and completing transfers safely.
Angeles shared, "Therapy has helped me gain strength. I learned so much. I even do exercises that I learned from therapy here in the room by myself. The therapist Sydney has been so kind and supportive. She always cheers me on and doesn't force me. I am very happy with her."
